Are you the legal owner of this dog(s) I give Green Acres B & B Boarding consent to administer my dog(s) medication I consent to have my dog(s) housed in a crate I consent to have my dog(s) co-mingled with other dogs I consent to my dog(s) wearing a citronella/electric bark collar I give Green Acres B & B consent to provide medical care to my dog(s) should an emergency arise I give perimission for my dog(s) to be on the sofa I give permission to use any photos taken of my dog(s) while boarding with Green Arcres B & B Boarding

In the unfortunate event that your dog passes away during its stay with us, please check your preference for the care of the body until you return.

-Green Acres B & B Boarding will use all precautions against injury, escape, or death of your pet
-All charges shall be paid upon release from Green Acres B & B Boarding.
-If a pet is not picked up within 48 hours of the designated time, the animal will be considered abandoned and may be dealt with as Green Acres B & B Boarding sees fit. It is understood that this does not relieve you from paying all cost before and up to the 48 hour notice.
